At the first appointment, new patients are asked to fill out a registration and medical history. Links to these forms are available below allowing you to fill them out in advance. A comprehensive dental exam is performed on the first visit which will give our dentists a baseline starting point for your ongoing dental care. This exam will also include diagnostic x-rays and oral cancer screening. Then, in consultation with the dentist, you will be able to decide on the treatment plan best suited to your needs.
